Name Window help?

Okay, I learnt this probably 2 years ago and I forgot due to lack of practice

Well I wanna convert the original WWF RAW's Name Window into the ones which are in Wiggo's patches [starting from Total Edition] . So if any of you know how to do it, please do reply
