4 player converted Canadian Destroyer?

Does anyone have a 4 player converted Canadian Destroyer? I tried converting a Canadian Destroyer code to 4 player with WldFb's Move Hack Converter OpenOffice document, but it didn't work, as the table confuses me.

I do believe thee should be one in TNA vs ROH. Not certain, though, it's been years since I played it.
